We are a telehealth startup. Patient video visits run through a third-party API, session notes are stored in a cloud Postgres database, and clinicians access records via a web app with email/password login. PHI is sometimes emailed to patients as PDFs. Backups are stored in cloud object storage. We have not yet signed BAAs with all vendors.

First: HIPAA generally applies if you bill insurance electronically or act for a covered entity. If you only take cash and never transmit claims, you may fall outside HIPAA - but state privacy laws still apply.

Regardless of status, fix these:
1. Unlocked filing cabinet - Lock all paper records containing health info.
2. Shared cloud drive - Restrict access to named staff; avoid broad sharing links.
3. Add a clear privacy notice describing how client data is used.
